The Health Information Management Department is responsible for handling patients’ confidential and personal health information in a conscientious manner that at all times safeguards and protects the patient’s right to privacy in accordance with hospital policy and HIPAA statues.
How do I obtain a copy of my medical records?
An Authorization to Release Medical Information must be completed and signed by the patient or their legal representative in order to obtain a copy of their medical records or to have them sent to another facility. There are three options to obtain a copy of this form:
- A copy of the form can be picked up in the Health Information Management Department.
- You can request a copy of this form be mailed or faxed to you by calling 207-795-2480 and speaking to a Release of Information Representative.

How long will it take for my request to be processed?
Depending on the specifics of the information being requested, processing times may vary. Most requests can be filled within seven (7) business days.
Is there a fee for copies of my medical records?
In some instances there may be a fee associated for copying protected health information. The exact amount cannot be determined until the request has been completed. Should there be a fee, the patient will receive an invoice mailed to their home address.
