A patient centered "medical home" is an approach to providing primary care to people of all ages. It is not a building or place, but a team of health professionals who work together to provide a central point for coordinating care to help people become as healthy as possible. Central Maine Family Practice is committed to educating families and health professionals alike. Our providers are members of the American Academy of Family Practice and the National Committee for Quality Assurance. In addition we participate in Joint Commission Accreditation Health Organization (JCAHO), Account Care Organizations (ACO), Center for Medicare and Medicaid Services (CMS) and other regulatory organizations. How Best to Prepare for Your Appointment: New Patients Bring any past medical records that have not already been processed to us. Arrive 15 minutes early to allow time for registration process. Follow-up or Routine Appointments Please ensure you have completed any required or preordered lab work so the provider can review your results and create a plan with you at the time of the visit. Complete any patient sections of forms being requested for the provider to fill out.
