Bridgton Hospital’s Diabetes Self-Management Program has been accredited by the Association of Diabetes Care and Education Specialists. It is the only outpatient, comprehensive diabetes self-management program in the region.
This accreditation represents a high level of quality and service to the community and the ability to meet the needs of Medicare recipients and others affected by diabetes in their region.
Bridgton Hospital’s self management program is designed to give patients information about diabetes and help them manage their diabetes. The course also introduces patients to a diabetes support system. You can find more information about it here.
Diabetes education services use a collaborative process that allows people with or at risk for diabetes to work with a diabetes educator to receive individualized care to help them reach their health goals. Diabetes education is a covered Medicare benefit when delivered through an accredited program.
